Please note that as of October 1, 2023 Medicare requires that clients sign a Scope of Appointment form at least 48 hours before an appointment can take place. This does not obligate you in any way to make changes to your current Medicare coverage. We will send an electronic form when you schedule your appointment but you may also print this form and sign manually. Again, this form must be signed & dated 2 days before our appointment!
